The information in this application is required to ensure you are suitable to contract with the Company to provide security services.

If you do provide services to the Company you will do so as a casual worker and not as an employee. For the avoidance of any doubt this means that your employment rights are limited to that which workers are entitled to. In particular, it does not create any obligation on the Company to provide work to you and by signing this document you confirm your understanding that the Company makes no promise or guarantee of a minimum level of work to you. In recognition of the fact that it is the intention of both you and the Company that there be no mutuality of obligation between the parties at any time when you are not performing an assignment of security services for the Company, we further acknowledge that you are under no obligation to accept worked offered to you by the Company. If you do choose to work with the Company each assignment shall constitute a separate contract for services. If you accept an assignment of work but are subsequently unable to complete it for any reason, you should inform the Company immediately.

You will only be paid for the hours that you work. The Company will make all necessary deductions from your pay as required by law and shall be entitled to deduct from your pay or other payments due to you any money which you may owe to the Company at any time.

As a casual worker your holiday entitlement will depend on the number of hours that you actually work and be pro-rated on the basis of a full-time entitlement of 5.6 weeks holiday (including all bank holiday entitlements) during each full holiday year. Your holiday entitlement is therefore equivalent to 12.07% of the hours that you actually work in each holiday year. At the end of each assignment the Company will pay you in lieu of any accrued but untaken holiday for the holiday year in which the assignment ends.

Criminal Background Check

You are applying for a position of trust and in the event of being offered employment by the company we may apply for criminal background check. However, having a criminal record does not necessarily bar you from employment. Criminal record information is treated in a sensitive way and is restricted to those who need to see it. By signing this document, upon request you agree to provide a copy of the criminal record certificates. The criminal background information is not retained. Screening

You agree to be being screened in accordance with British Standard (BS) 7858 and will provide information as required. Any offer of employment is subject to satisfactory screening, and you acknowledge that any false statements or omissions could lead to termination of employment.

I further declare that any documents that I provide as proof of my identity, proof of my address, and any other documents that I provide are genuine and that any falsified documents may be reported to the appropriate authority. I understand that it may be a criminal offence to attempt to obtain employment by deception and that any misrepresentation, omission of a material fact or deception will cause for immediate withdrawal of any offer of employment.

I accept that I may be required to undergo a medical examination were requested by the company. I understand and agree that if required, I will make a statutory declaration in accordance with the provisions of the Statutory Declarations Act 1835, in conformation of previous employment or unemployment.

I understand that any false statement or omission to the company or its representatives may render me liable to dismissal without notice.

By signing this declaration, I agree that I have provided complete and true information and I understand that knowingly making a false statement for this purpose may be a criminal offence.

I understand that Assure & Secure will protect my personal data in accordance with the General Data Protection Regulations.
